Correction/Amendement actions in LSM report

SUMMARY:

Lab Service Manager (LSM) add-on is the perfect application for service core & test labs.

It also helps you to create & customize templates for reports when a job is finished. You can easily export a report in PDF format as soon as you complete a job (test).

Moreover, you can use the “dynamic fields” to enter in your report templates. These dynamic fields will help you insert automatically the values/results/patient/sample, etc information, when a job is complete. You have dynamic fields for:

  • Results template / Processing template

  • Invoice template

  • CoC report template

  • Batch report template

  • Dynamic tags

All of them are described in our Dynamic tags KB.

To imagine a scenario, where there can be errors in the reporting for a test. For example, sometimes the patient or sample information are incorrect and the lab wants to generate a new report with the corrected version. However, to be more compliant with the regulatory bodies it is imperative to mention clearly the changes done to an already existing report. For this very purpose, LSM now offers the possibility to add the changes in the already finalized report. This can be helpful to be more compliant with CAP (College of American Pathologists) and CLIA (Clinical Laboratory Improvement Amendments) regulations. Please follow the below steps to see how to add these changes in LSM:

Please note

The amendment and corrective can only happen when the job is in “Finished” status. They will NOT be recorded in the report if they are being done on a job with “Completed status”.

Explanations

Amendment/Revised: An amended or revised report is issued when the final diagnosis changes or other important pathologic information becomes available. The reasons for the revision must be explained in the report and the clinician(s) notified, because a revised report may significantly affect patient care. 

Corrected: Some departments also define a corrected report, which is issued when transcription, patient identification, specimen site, or other related reporting errors occur. A corrected report differs from a revised report because the final diagnosis remains unchanged. Corrected reports should be clearly identified and the reasons for the correction included in the report. The clinician(s) should be notified when corrections are likely to influence patient care. Corrective date in LSM means the date when the job was corrected.

1. Adding amendment and correction tags in report

  • In order to create a report you need to go to LSM -> ADMIN -> PREFERENCES -> REPORTS & INVOICE TEMPLATES.

  • You can add various dynamic tags in to your report templates that are populated automatically when the final report is created.

  • To add the amendment dynamic tags go to LSM -> ADMIN -> PREFERENCES -> REPORTS & INVOICE TEMPLATES.
    *Below is a pre-made example template and in yellow you can see the amendment title, amendment comment, corrective title, corrective comment and corrective date.

2. Adding Amendment

  • You can create a job in LSM or add it by LSMRemote when a provider send you the job request or samples.

  • A job in LSM has 4 stages. For the tags to work the job amended should be in “Approved” status. (Finished job)

  • You can filter the jobs to see the finished (Approved) jobs.

  • You can filter the results according to the status and search like step 1,2 & 3.
  • 4. Once you finished the jobs like in above image. Click on the options icon .
  • 5. & 6. Go to Manage ->Validations
  • 7. Once you click on validation you will see a pop-up to either restart or correct the job.

  • 8. You will need to add the test for the restarted job.
  • 9. Once test is added, click on save which will open a pop-up to add the reason for restart. This will be the “amendment_comment”
  • 10. & 11. Add the comment or reason to restart and save it.
  • 12. This job will be then visible in LSM -> JOB -> JOB LIST


  • You can execute the job. *Please read the KB to know how to start a job and create results.
  • Once you reach the Finished/Approved state, you can filter the job as above step 1 and check the report by going to OPTIONS-> REPORT-> PDF.

 

Please note

The amendment and corrective can only happen in LSM add-on. You cannot do it from LSMRemote. You can request your client/test lab to tell them which job needs to be amended or corrected.

3. Adding Correction

    • You can follow the same steps as mentioned above in “Amendment using LSM add-on” until the validations which provide you with “Restart” or “Correct”.
  • When you click on correct, you will see a pop-up to add the correction.

    Please note
    The corrections need to be done by going in the module in LabCollector. For example if there is a module called patient for test labs, which is connected to the case records in LSM, then the correction can be done for the patient details inside the patient module.
  • 1. 2. & 3. You can write the correction you performed and click on submit.
  • 4. After clicking on submit, you will be able to see comment you added in red along with the “corrective date”, meaning the date when the correction was done. 

  • The “Correction comment” and “corrective date” will be reflected in your report.
    *Please remember to add the tags in the report as shown in section 1.

3. Adding append comment

  • In a scenario where the results are “Invalid” or “undetected” or “inconclusive”, there can be a need to add the reason for such result.
  • To address this purpose we have the tag ##parameter_name_comment## which can be used to add comment to any parameter when you execute a test in a job.
  • These can be parameters at the processing or result type parameters at the test level.

  • So the part ##parameter_name_comment## can be replaced with names of the parameters. Example, ##Viability_comment## or ##RT-qPCR COVID 19_comment##.
  • The comments will be reflect in the report if the tags are used in it.
